Official Certificates 

The Chiyoda Campus Administration Office issues official certificates for alumni. These include certificates of attendance, completion and withdrawal. To request a certificate, please complete one of the forms below and hand-deliver or post to the Chiyoda Campus Administration Office. (The Office does not generally accept requests by phone, fax or email.) Certificates are provided for free.

Former students who studied at the following departments may apply for a certificate:

In-Person Applications

To apply in person, please bring proof of identification (such as a driver’s license, health insurance certificate, passport or foreign resident’s card) to the Chiyoda Campus Administration Office (Level 5).

If someone will come on your behalf, make sure this person brings a signed authorization by the applicant (any format will do) and proof of identification of the agent. 

It may not be possible to issue the certificate on the spot. You may wish to bring a stamped, self-addressed envelope just in case. 

Office hours are 10:00-18:30 week days (except during end-of-year and mid-summer holidays). 

Postal Applications

To apply via post, please include the following three items. The Administration Office will issue your certificate within three days of receipt of your postal application, although special or older certificates may take longer so please apply well in advance.
